SUBMITTING A SCRATCH FORM.

Fill out the Scratch Request form

Give or email to Hunt Test Secretary to sign.

HT Secretary will sign and email back or can take a photo of signed form.

Submit the signed form and fill in refund request information through the website reimbursement module.

APLA Treasurer will verify with Secretary, approve, or disapprove and send out check.

PLEASE SUBMIT AS QUICKLY AS POSSIBLE SO WE CAN KEEP ACCURATE RECORDS

SUBMITTING SCRATCH FORM THROUGH WEBSITE REIMBURSEMENT MODULE.

Once you have filled out the form and had it signed by the Secretary, you must submit it through the Website Reimbursement module to get paid. The following are instructions to do this.

SCRATCH REQUESTS MUST BE SUBMITTED SEPARATELY FROM OTHER REIMBURSEMENT REQUESTS.
YOU MAY SUBMIT MULTIPLE SCRATCHES IF THE CHECK IS BEING PAID TO YOU.



To access to the Expense Reimbursement Module do the following.


Login to the Website

Click the black Circle to the right of your name in the upper right hand corner.



Click the black triangle next to your name.



Click [Profile]

In the Profile Page look for the "Histories" section.


Select the [Expense Reimbursements].

In the [Title Box] write the following
Scratch Request


In the [subgroup] box select the

American Pointing Labrador Association.


Next to the  [project] box, click [Select Event].

DO NOT WRITE IN THE PROJECT BOX.
CLICK THE FAR RIGHT [V] ABOVE TO SELECT THE CORRECT HUNT TEST.

In the [payto] and following address boxes, put in the person's name and address who will be receiving the check

Select the date in the [date box]

In [vendor] box type scratch

In the [description] write in your dog's name without titles ex. [Birchcreek Cowboy Tiger Lilly Mac]

In the [category] box select Scratch Entry Refund

In the [amount] box enter the amount you are refunding.

Click [add]

Go to bottom and click [save pending] to save this information. Do this before you add any supporting document.

Get back in the pending request and add your Completed Scratch form and any other supporting documents you may have.

Review all of your information and click submit.  If you made a mistake contact tswanson.APLA@gmail.com and he will reset your submission, so you can amend it.
